Hi,
This is easier to understand if we separate some concepts:

Making bitmap images:
The real draw sources can optionally be installed when with MSM (I think that's what you found). These allow you to use the bitmap sources to make new button bitmaps etc. So one uses realdraw to create and save the desired bitmaps for buttons and panels etc.

Placing things on screens:
To add a button to a screen, I suggest using Machscreen. Machscreen is the program used to make the MSM screens and it does the job of placing buttons, DROs, images etc. where you want them on a page. So that you don't have to start from scratch, you can open an existing MSM screen set and modify it in Machscreen.

(Note: an alternative to Machscreen is the old, unmaintained screen4 program. I recommend NOT using that as there are known cases where it will corrupt a set file's internal data structure. Since Screen4 is no longer supported by Artsoft, once that happens you are "up the creek" - use Machscreen instead.)

Getting an MSM screen set copy to modify:
You don't want to modify the MSM screen set files directly; as they will be overwritten by the next MSM install. So MSM provides a way to clone an existing MSM screen set: the "Clone Screen" button on the setting page. This makes a copy of a stock (or previously customized) MSM screen set. You can then modify the copy to your hearts content without worrying about an MSM install stepping on your work later.
Details of this MSM screen customization process are in section 15 of the MSM mill user manual.
